This is a pussy and click chastity which, I believe, belonged to Alain (aka Alarac) of Misty and Alarac of France.
Misty had 4 piercings per outer pussy lip and a vertical clit hood piercing (among other piercings) So this chastity device was fitted through those piercings. The balls at the ends of the barbells are threaded - they are standard, large gauge, barbells. The vertical cast round cylindrical piece with the holes in it was for insertion into the pussy, originally to assist with urination, and to provide for some penetration while being worn. As I understand it, this vertical cast piece did not work out too well, as in practice its diameter was to small and it was to straight and stiff.
There are images of Misty wearing this in old Misty archive pictures.

I have not seen Misty wearing this, but this is a clit hood cover that Alain had made for her, which is on a similar fixing.
